Appeal Form

This form is used to submit an appeal to the National Healthcare Certification Organization regarding certification eligibility. Appeals must be submitted within 30 days of the notification date of the decision or transaction being appealed. Applicants should be prepared to upload any supporting documentation relevant to their appeal.

Supporting documents may include:

• Copy of accommodations denial notification
• Copy of disciplinary notification
• Copy of exam, certification, or recertification denial notification
• Letter of recommendation
• Proof of graduation

ⓘ Pass/fail examination scores are not appealable and are addressed under the Complaint Policy.

Candidates will receive a response via email within 30 days of submitting an Appeal Form. If any additional documentation is required to complete the review process, candidates will be informed via email.

The Appeal Form fee is non-refundable.
