This Renewal Application is used to request reissuing of an individual’s Certified Phlebotomy Technician (CPT) certificate that has been expired for more than 1 day and up to 2 years. If you need assistance to find your current certificate information, please utilize the tool found on the https://www.nhcocert.com/verification/website.
Individuals should be prepared to submit supporting documentation showing 30 hours continuing education completed with an approved site. Examples of approved sites include:

We encourage you to submit your application as soon as possible. Please keep in mind that you have 60 days from the date of enrollment/purchase to submit it and have it approved. This countdown begins on the date you purchased the application.
Submitting early helps ensure ample time for processing and approval.
Renewal guidelines include:
- Applications will be reviewed within seven (7) business days and an approval or denial notification will be sent to candidates via email.
- If renewal requirements are not met, candidates will have thirty (30) days from the denial notification to address outstanding requirements.
- Approved reissued certificates will be sent via email and the digital badge will always be available in the candidate’s online NHCO portal.
- Renewal denials can be appealed within thirty (30) days of notification.
ⓘ Candidates who submit all required supporting documentation will receive a digital, printable certificate via email. The CPT badge certificate will also be available in the candidate’s online NHCO portal.
The CPT Renewal Application fee is non-refundable.
